Job Summary:


The One Door Family Shelter Diversion Specialist provides assessment services and case management support for those experiencing or at-risk of homelessness, focusing on housing problem solving and homeless prevention aimed at diverting households from the emergency shelter system. The position is located at the O’Reilly Center for Hope.

Evaluation:


The New Day Assistant Director, following adopted policies of Community Partnership of the Ozarks, will evaluate the One Door Family Shelter Diversion Specialist.

Reporting Structure:


The One Door Family Shelter Diversion Specialist will report to the New Day Assistant Director.

Duties and Responsibilities:


  • Meets with clients to complete screenings and assessments to determine need and program eligibility;
  • Works with clients to establish action plans to increase housing and financial stability;
  • Engages clients in problem-solving dialogue to assess for additional resources a client may be able to access within their network;
  • Connects clients with appropriate partner agencies through referral and follows up to ensure connections were made;
  • Works with landlords and property managers to identify and achieve permanent housing solutions;
  • Provides mediation assistance to encourage family reunification, conflict resolution, and landlord or other party reconciliation;
  • Maintains records of funds spent; ensures reporting is completed as specified by funding source;
  • Enters, updates, and maintains data in the Homeless Management Information System (HMIS);
  • Provides and facilitates crisis intervention assistance as needed;
  • Completes required documentation to ensure compliance and ascertain the effectiveness of diversion activities;
  • Tracks and reports progress and services for each client receiving diversion assistance and the project as a whole;
  • Serves as a “front-door” access point for families accessing the local Coordinated Entry System;
  • Advocates for clients’ needs;
  • Participates in CPO’s resource development/fundraising activities;
  • Driving is an essential job function of this job position;
  • Other duties as assigned.

Essential Qualifications/Skills:


  • Bachelor’s Degree in Social Work or other related field is required;
  • Proven written and oral communication skills;
  • Strong computer skills, including Microsoft Office and Internet;
  • Must be comfortable working with diverse populations, including high risk and homeless households;
  • Proven ability to work effectively in a fast-paced environment;
  • Detail oriented with strong organizational skills;
  • Must have a valid driver’s license.

Position Clarification:


The position of One Door Family Shelter Diversion Specialist is a grant-funded position and is based upon funding availability. The job duties are subject to change, based on the funding requirements. This is a non-exempt, full-time position. Grant funding dictates the pay range for this position. CPO receives federal grants and contracts and therefore must comply with the Drug-Free Workplace Act of 1988.
